If you have not been in our office in the last three years, complete the New Patient forms. If we’ve seen you within three years, choose Established Patient.

Records Release
We can share your health records directly with you four ways: via U.S. mail, via fax, via our patient portal, or in person. We cannot email records containing health information. If you would like us to share your records with another office or optical, you must complete records release form and submit it via fax or secure email.
